Coroner to see if lessons can be learned after Worcester care home death

An inquest ruled staff's neglect contributed to the death of 49-year-old Rachel Johnston in 2018

A coroner will today look to see if lessons can be learned following the death of a disabled woman at a care home in Worcester.

Rachel Johnston, 49, died at Pirton Grange care home on 2 November 2018 days after undergoing surgery to have several decaying teeth removed.

She was discharged from Kidderminster Hospital on October 26, 2018 however began bleeding and died a week later from a hypoxic brain injury.

On February 11, 2021, the county coroner concluded her death was contributed to by care home staff's neglect.

Today (March 26) the coroner will consider a prevention of future deaths report and set recommendations to prevent this happening in the future.
